A Thunder Bay long-term care facility has been reprimanded by the provincial government over a number of issues, including abuse and neglect of residents. Home, Community and Residential Care providers are the local organizations established by the Ministry of Health and Long-Term Care that provide access to government-funded home and community services, including admission into LTC homes. Home, Community and Residential Care have Case Managers/Placement Coordinators who authorize admissions into LTC homes (for both permanent and short-stay admissions) and arrange for home care services. After a complaint about staffing shortages, the Ministry noted many records were incomplete, as they were unable to be filled out by staff during a shift.

One resident, who was interviewed by a Ministry inspector noted they were only given a bath once a week over a one-month period.
